Notice for Provision of "INITIAL" Services & Programs form REVISED January 2012 (need at Initial IEPs only)
Consent to Invite Community Agencies (Use this form, prior to sending out the invitation to IEP, for anyone not part the school district--outside agencies such as CMH, Michigan Rehab, etc. Parents can invite anyone they want...you have to seek parental permission to invite any outside agency to the IEP.)
Excusal from IEP (MUST have permission from parent to be absent from an IEP. This is highly discouraged, but emergencies happen. Use this wisely.)
