Business & Work Related Forms:

• Alter Work day forms.doc, Use this form when your work day is longer than usual for unusual circumstances (i.e., field trip)
• Conference Form.pdf, To be used whenever you attend a "requested and approved" conference
• FieldTrips-Reimbursement.pdf To be used when you pay for the field trip and need to be reimbursed. Be sure to get the appropriate signatures.
• Photo/Video Parent Permission (form)
• Substitute Staff Input Form (To be used when you are absent. Provides feedback as to how well the substitute did in your absence.)
