FORMS

• Summary of Performance:This form must be completed for students who are graduating with a general education diploma or certificate of completion, or who are leaving school because they exceed the age of eligibility. An "exit" IEP is required.
• Summary of Performance (PDF link: use this form if you do not have Word; you need to be short & sweet)
• Summary of Performance (Use this link if you have access to Microsoft Word and need a lot of room to write.) 
• Consent to Invite Community Agencies  (Use this form, prior to sending out the invitation to IEP, for anyone not part the school district--outside agencies such as CMH, Michigan Rehab, etc. Parents can invite anyone they want...you have to seek parental permission to invite any outside agency to the IEP.)
